Levels Of Care


Level 1 Services:
• Minimal supervision with bathing and grooming
• Medication reminders
• 24 hour nursing staff available / call systems
• Personal laundry
• Daily bed making
• Safety checks
• meal and activity reminders
• Weekly blood pressure and weight documentation
• One item from list of level 2 services


Level 2 Services: (Includes all level 1 services plus 2-5 of the following...)
• Medication set-up
• Administration of medication (does not include injections or nebulizer treatments)
• Standby assistance with dressing
• Assistance with bathing two time a week
• Assistance with personal hygiene
• Morning and evening preparation
• Assistance with coordinating medical appointments
• Toileting (including assistance w/pads)
• Bed changing in excess of once a week
• Assistance with transfers and ambulation on a regular basis
• Assistance with telephone on a regular basis
• One hour or more per week in re-direction counseling with resident and/or family or behavioral intervention
• Routine injections
• Diabetes management (accu checks and insulin set-up)
• Routine lab draws
• Catheter care
• Oxygen management
• Daily weight and blood pressure
• Pacemaker management
• Routinetreatment orders
• Routine nebulizer treatments
• Personal shopping
• Transfer assistance on a regular basis (resident must be able to stand, pivot, and bear weight)
• Physical assistance with eating/special diet
• Assistance with picking up or delivering personal mail to and from mailboxes
• Bathing in addition to twice weekly
